Payment Terms/Agreement/Release and Refund Policy

I understand that full payment for classes is due upon registration.  Once your reservation is confirmed by payment, our refund policies apply.   At that point you own only the dates, times, and services you have paid for and specified.    Transferring to another start date will only be permitted if space allows and the dog passes all requirements to attend.  Full payment and any coupons are to be presented upon registration, one coupon per family.

Aggressive dogs are not permitted and if Lynda O’Neill feels that the dog is a risk to other dogs or people, the dog will be removed immediately and absolutely no refund will apply.

The following refund policy is in effect as of November 16,2011.

Refund Policy for Puppy 1:  No refunds will be accepted for the Puppy 1 course unless cancellation occurs at least 7 days prior to the start date of class.  If a person must cancel the Puppy 1 course with no less than 7 days notice before the starting date a full refund of $160.00 minus a $25 processing fee will apply. Once manuals have been handed out and classes have started, absolutely no refunds will be given.

Puppy 2 cancelled no less than thirty days before the starting date will receive a full refund of $200.00 minus a $25 processing fee.  If a refund is applied to the Complete 12 week Puppy Course with no less than 30 days notice before the start date, it will be subject to a refund of $175.00 minus a $25.00 processing fee.

Puppy 2 cancelled between thirty days and twenty one days before the starting date will receive a 50% refund of $200.00 minus a $25 processing fee.  If a refund is applied to the Complete 12 week Puppy Course cancelled between thirty days and twenty one days before the start date, it will be subject to a 50% refund of $175.00 minus a $25.00 processing fee.

Puppy 2 cancelled less than twenty one days before the starting date or after the starting date will receive no refund.   If the Complete 12 week Puppy Course is cancelled with less than twenty one days notice before the starting date or after the starting date, no refund will apply.

If Lynda O'Neill can not provide an appropriate Puppy 2 class (for whatever reason) when a client pays for the Complete Course up front, a complete refund of $175.00 will be given.  The client is then paying the regular rate of $160.00 for Puppy 1.  Client may be offered 2 start dates at Lynda O'Neill's discretion.  Restrictions do apply.

I understand that I will be held legally responsible for all NSF cheques and the applicable charges. A service charge of $45 will be charged on all NSF cheques. We reserve the right to certify any and all cheques at your bank before processing. All non-payments will be sent to collections after 60 days unless other arrangements have been made.

I understand that there will be a replacement cost of $35.00 /manual, $4.00 /clicker and $2.00/20 ft. leash if course content is withheld from return, lost or stolen. I am aware of Canadian Copyright Laws.

Disclaimer:

The 20 ft leash that may be supplied in your training package is intended for you to practice the "recall" command at home.  Do not rely on this leash as a life line.  Do not use this leash outside of a secured area.  Doing so may result in injury.

Class Attendance/Transfer Policy:

Class attendance is the responsibility of the owner. Classes and/or payments are non-transferable and no credit vouchers will be issued should a client miss a class or drop out of a program. If you must miss a class, contact your instructor and check your manual to see what exercises you will need to work on and I will then do my best to catch you up at the next class. Should you wish to schedule a private session to catch up, arrangement can be made and a private training fee will incur.

Safety Precautions Agreement:

Make sure all equipment you are using (collars, leashes, head collars, harnesses) fit properly to ensure that your dog does not get loose in the parking lot, or in class.  Choke chains, pinch collars or shock equipment are not permitted on any dog for any reason.

All dogs must be confined to a leash at all times unless instructed otherwise.

Be careful when opening the door to your vehicle without first checking that equipment is secure and you have a good grip on your leash.

Never open the door to enter or exit the school without first notifying the instructor. We have off leash playtime in the yard; weather permitting it may be in the building. The door may be locked during class time to ensure everyone is safe.

I understand that when training in an indoor/outdoor venue that there are risks of injury such as, tripping or slipping, fence jumping, running into obstacles and acts of nature. I agree that I will take full responsibility for these risks.

Health and Vaccination Agreement:

I understand that it is mandatory that my puppy has received at least one set of vaccinations and has been in my home for one week prior to starting puppy classes.

I agree to provide the instructor with proof of vaccination, spay/neuter certificate (Puppy 2) on or before the start date of my class.

If proof of vaccination, spay/neuter certificate (Puppy 2) is not given to the instructor on or before the start date of class, I understand that my dog may not be able to participate in that class.

Signs of sickness may include, but is not limited to coughing, vomiting, diarrhea, choking, lose of appetite or depression and I agree that I will not bring my dog to class if I suspect he/she is not feeling well.

I understand that anywhere dogs meet there is a risk of contracting a sickness. Kennel Cough is highly contagious and can be contracted by sniffing an area where another infected dog had been visiting. The animal can be contagious without ever showing any symptoms, and I am completely responsible for this risk.

Release from Liability:

Declaration:  I declare that dogs can be unpredictable and as such have the potential of displaying unwanted behaviours without any warning. I acknowledge and understand that dogs can carry and contract transmittable diseases regardless of vaccinations. Potential concern arises from exposure to disease prior to vaccination or possibly vaccination failure. Additional concerns arise from declining optional vaccinations/preventative medications including, but not limited to: Kennel Cough (Bordetella), Leptosporosis, internal/external parasites, (giardia, coccidiosis, etc.) I also acknowledge and understand that dogs can have or develop a variety of allergies, which may be related to food and/or environmental sources and will not hold Confident Canines Companion Training and or Lynda or Jamie O’Neill responsible for any incidences of disease, parasites or allergies.

Release from Liability:  Due to the fact that animal behaviour is unpredictable, I agree to hold harmless and release from Liability, Confident Canines Companion Training, and Lynda and/or Jamie O’Neill for any and all risks involved with Confident Canines Companion Training for any and all direct or indirect results.

Indemnity: I agree that I am assuming all risks and releasing the above listed names and business as a result of any and all collection, insurance claim, lawsuit or action that is the result of the behavior or actions of dog, anyone, or myself that is in my care.

Assumption of Risk:  I understand that working with dogs is not without risk to other persons, dogs, other possessions or me.

I acknowledge that I fully understand the terms and conditions provided on this document and by signing this document, I am doing so, voluntarily and unconditionally.
